Some have lower back pain, others mid, and still others neck pain. The consistent issue with all of these patients is imbalance in their spine. Whether the condition be due to poor posture, trauma, or hereditary factors, the fact remains that back pain can interrupt a person's life and lifestyle. During the last 27 years of active chiropractic practice I have utilized a variety of treatment techniques that have consistently proven effective in treating the majority of spinal conditions that have entered my office shoe insoles. Always looking for better and more efficient ways to help patients led me to direct my attention to support the spine AFTER the patient had received their treatment. In the past I had seen orthotics and knew that there was a benefit to supporting a patient's arches. Let's go over a bit of anatomy to understand the benefits of orthotics. Your foot has not one, but three arches. The third arch is called the transverse arch. It is located across the bottom of your toes, sometimes called the ball of your foot. All three of these arches work together to support the foot. In turn the foot supports the ankles, knees, and eventually the spine. As we get older or if we continue to wear shoes that are not very supportive, the arches have a tendency to drop. As the arch drops the support required diminishes and affects the entire structure. Typically, the outside arch is the first to start dropping. Many professionials that make customized orthotics still use old technology to make the support. The patient steps into a foam cast which is then sent to the orthotic company for production of the orthotic. The problem that I have seen is that it is difficult if not impossible to make a perfect cast using this old technology. Thankfully, technology has developed that digitizes the process. Use of an electronic digital scanner takes multiple measurements to give the most accurate casting procedure possible. In my office this has allowed patients who previously had chronic lower back pain, to finally get the support their conditions required. Basically the price difference depends on the manufacturing process. Years ago the orthotic arch supports were crafted by hand and took many hours for a skilled technician to make. The doctor would make a cast of the patient's foot with plaster. They would then send the plaster mold off to a lab to manufacture the orthotic. There still are some labs that make the devices by hand but these are becoming a thing of the past. Today there are ways to manufacture custom arch supports that are much less labor intensive. Lasers and computers are used to make a 3-dimensional computer image of the foot. This image is then sent to a computer controlled milling machine which automatically machines out the orthotic. This process is less labor intensive but the computers and machines to do this are expensive. Therefore although the technology speeds up the process it does not bring down the cost appreciably. To make the custom arch supports more affordable many labs now use a set of 100 or so pre-manufactured orthotics. The pre-made arch supports are sized according to length, heel width and arch heights. The shapes of the pre-fabs were based on the foot molds of over 100,000 patients. The most frequent shapes were mass produced to have a set of custom arch supports that would meet the needs of 70% of the patients. For most of the other 30%, if an adjustment is needed to fit the orthotics exactly to the patient's mold, the support can be heated and formed to conform to the mold. In choosing between both types, you have to consider the pros against the cons. The list of pros for the insert shoe lift is that you can use it with just about any of your current shoes, its cheaper, and you are not limited to the styles that the makers of custom shoe lifts put out. The biggest con is that you are limited to about two inches or less of added height appearance. Custom shoes have the pro of the potential for added inches, but they are much more expensive and your selection of shoes is dependent on the styles that the manufacturer makes. That means that you can no longer buy and wear any conventional shoe that you see and like because you are now limited to wearing shoe lifts indefinitely because you have publicly shown the world that you are falsely two to four inches taller than you really are.
